The care coordination program uses a client-centered, collaborative approach to address client care, which helps establish the foundation for a healthy and productive partnership. Care coordinators support the achievement of client goals by providing tailored care and expertise throughout all phases of living with HIV.
Care coordinators can also help address a variety of needs, most of which can be met directly by Tulsa CARES or through outside referrals.
Care coordination staff assist clients with completing necessary paperwork for Social Security, Medicaid, and Medicare as well as screening and enrollment of Supplemental Nutrition Assistance Program (SNAP) benefits.
The HIV Drug Assistance Program (HDAP) provides medications to eligible individuals when no other pay source is available. Tulsa CARES also maintains agreements with local pharmacies and offers co-pay assistance to eligible clients.
Clients who experience transportation barrier may be eligible for transportation assistance, which includes gas cards and Tulsa Transit passes (bus and The Lift).
If you’re not requiring care coordination or other Tulsa CARES programs, we offer health care navigation.
These services maneuver a complicated health care system on behalf of clients and are ideal for those who only need assistance with obtaining or renewing insurance coverage or application support for the HIV Drug Assistance Program (HDAP).
